Back in December of 2004 our friend Dr. jay Stern closed his long time practice here on Dorsey Hall Drive. This was a difficult and personally troubling move for Dr. stern to make but he felt that it was the correct thing to do at the time. Dr. Stern as most of you already are aware asked me to open my door to his former patients so that they would not be in the lurch concerning their healthcare needs. Some of you have raised questions concerning your medical records and what steps you need to take to obtain the old records from Dr. Stern. You have as a patient three options to obtain your medical records from Dr. Stern for future healthcare needs. The 1st and easiest way is to simply sign a record release form here in our office during your initial visit with us. This will allow Dr. Stern to release the records to my facility, the copies we obtain are your property and should you ever choose to leave our practice they would be your right to take them with you (at no cost) as well as copies of anything we produced. Your second option is to write to Dr. Stern directly @ Dr. Jay Stern 5072 Dorsey Hall Drive Suite 103 Ellicott City, MD 21042 In your letter you will need to put a contact phone number, a mailing address you wish the records to be transfered to, your date of birth and of course your name. Please make sure to sign the letter. Your final option should you be reluctant to have Dr. Stern transfer your records is to wait. Dr. Stern must by Maryland Law keep copies of your records and make them available to you for a duration of 7 years following the closure of his practice. For those patients who have taken these steps aleady and are currently curious about the status of the transfer, as of 2-22-2005, we have NOT recieved ANY records from Dr. Stern and have made efforts to contact him to help speed the process up. Lastly, many patients are concerned about Dr. Stern's personal health and or well being. While I do not have the details on Dr. Stern's situation we would like to pass along to you that Dr. Stern is personally healthy and doing well.
